Regular general health examination for employees of our school in 2024
Time:Tuesday, August 13, 2024, 8:00 a.m. to 11:30 a.m. (last registration time).
Location:Exhibition Hall 210 (Registration Office) and Lecture Hall 202, 2F, Complex Building II, Heping Campus.

Target:
- Colleagues who have completed the registration and received an email notification from the Human Resources Office that they are eligible for the health examination.
- For questions about age and seniority for publicly funded health checkups, please contact the Human Resources Office (Ms. Chou 1305).
- Voluntary participants at their own expense, please register with the Human Resources Office (Ms. Chou 1305).
- For health examination questions, please contact the Health Center (Nurse Liu 5754).
- For any funding issues, please contact the Accounting Office (Ms. Feng from the Administrative Unit, 1172; Ms. Liu from the Academic Unit, 1152; project personnel please contact the respective project organizers).
Notes
- For publicly-funded health check-ups, please complete the submission of the health examination report (original) and application form of other hospitals before 113/10/15 (the application form needs to be signed and approved) to facilitate the verification process. Please be aware that document cannot be accepted after the deadline. If you are unable to make the deadline, please notify us as soon as possible.by October 15, 2024The application form must be signed and approved to facilitate the verification process.Late submissions will not be accepted. - 請完成報名後，請盡速完成在職勞工一般健康檢查補助申請(申請表)，下載位置：校務行政系統→人事相關系統→健康檢查補助申請系統，簽章並經主管、相關單位(約用人員、專任計畫助理、參加公保之教職員及參加勞保之技工工友請送人事室，全職臨時工及全職工讀生請送總務處事務組)及主計室核章後，送交學生事務處健康中心彙整。

- 國民健康署癌症篩檢(糞便潛血檢查、乳房攝影、子宮頸癌抹片篩檢)會與113年度在職勞工定期一般健康檢查合併在113年8月13日(星期二) 上午辦理，請有興趣的同仁與健康中心劉護理師聯絡(#5754).
