National Taiwan Normal University New Students' Health Examination for the 113th Academic Year 1130425 Revised
According to the regulations set forth by the Ministry of Education, new students are required to undergo a health examination upon admission. As outlined in the "National Taiwan Normal University New Student' Health Examination Implementation Guidelines, students who do not complete the health examination as mandated will be considered to have not finalized their registration process. They must complete the health examination before they can receive their student ID.
★New students who wish to take a leave of absence during the academic year should refrain from participating in the school’s physical examination for that year. Instead, please complete the physical examination with the new students upon your return to school in September of the following year. (If you return to school in February 2015, please schedule an appointment for a physical examination and submit the report to the Health Center before your return.)
★★★★ Important reminder:
▲▲1. In addition to the health examination they underwent when applying for a residence permit, overseas Chinese and foreign students are still required to complete the new student' health examination, as the examination items for the two processes differ.
▲2.▲2. Labor general physical examination or military service physical examination report cannot be used as it does not comply with the items specified by the Ministry of Education.
★Student dormitory access control system: Access is by student ID card. Residents who have not yet undergone physical examination and received student ID card must obtain a temporary card at the dormitory to enter and exit.

Please follow the "New Student' Health examination Schedule" to schedule your visit to campus. *Bachelor students can collect their student ID cards on-site after completing the undergraduate physical examination. *Master and Doctoral students can go to the department office to collect their student ID card with the receipt of the on-campus new student' physical examination (if the department has announced other schedules for collecting student ID cards, the department’s announcement will prevail) |
Students who enroll early in the second semester, participate in the EMBA & GF-EMBA, summer classes,or are returning students may go to an off-campus hospital for a physical examination. |
1. For new students who are unable to attend the on-campus health checkup, please go to a qualified medical institution for a physical examination before the start of school (please download and print "National Taiwan Normal University Student Health Examination Form" and bring it to the hospital for a physical examination according to the prescribed items). Please allow time for the hospital's physical examination report, which is about 3 to 4 weeks). 2. Please provide a physical examination report within 3 months (e.g. if the school starts inFebruary 2025 ,you need to submit a physical examination reportafter November 2024 ).However, the content of the report must comply with all the items in the school's physical examination form. Please download the first page of the school's health information card, fill it out, and bind it to the cover of the physical examination report and submit it together. After submitting the off-campus physical examination report to the Health Center, the Health Center will issue a "Physical Examination Report Submission Certificate. you can collect your student ID card from the Academic Affairs Officeafter the semester begins,provided you present the physical examination payment receipt from either Lohas Clinic or Qixin Clinic.Graduate students should visit the Graduate Academic Affairs Office or the Academic Affairs Office at the Gongguan Campus (for students from Gongguan Campus) to collect their student ID cards.. |
★How to submit off-campus physical examination report:
Physical examination reports canbe submitted in person at the Health Center(either Heping Campus I or Gongguan Campus, see map for details), by mail (Health Center, No. 162, Section 1, Heping East Road, Taipei City), orby mail(Health Center, No. 162, Section 1, Heping East Road, Taipei City)by e-mail (anni1954@ntnu.edu.tw) .They must be submitted within 1 month after the start of school.Health Center working hours: Monday to Friday (8:30-17:00), there are staff on duty at noon,Please pay it to the Administrative Office of the Health Center on the 4th floor of Lohas Clinic( Heping Campus I) on Fridays during the winter and summer vacations.
★Notes for off-campus physical examination:
1. Fasting is not required for the health examination. Please maintain your regular schedule, avoid staying up late, and refrain from overeating for three days prior to the check-up to ensure accurate test results. Chest X-ray examination (wear a top without underwire or metal ornaments) is not recommended for pregnant women. Chest X-ray report must be submitted to the health center after delivery.
2. Local students please bring the original ID card, foreign students please bring the original and a copy of their passport, residence permit or entry permit.
3. Physical examination items: (All the following items must be completed. If there are any missing items, you must go to a hospital or clinic to make up for the missing items)
1) General examination – height, weight, waist circumference, pulse, blood pressure, vision, color vision, hearing,Oraland physical examination.
2) Blood test – liver function (SGPT, SGOT), creatinine, urea nitrogen (BUN), uric acid (UA), total cholesterol (T-CHOL), complete blood test (WBC, RBC, PLT, HCT, HB, MCV),Hepatitis B surface antigen antibody.
3) Urine test – urine protein, urine occult blood, urine sugar, pH value.
4) Chest X-ray.
