The appointment hotline is: (02) 7751-5688 ext. 3 or the switchboard<\/li>\n\n\n\n<li>Please register at Zhongzheng Hall of Gongguan Campus at your chosen time on August 20 (Tuesday). Please read the pre-departure notice and the precautions for the health checkup before registering, and complete the \"Online Questionnaire for Special Hazardous Operations of the Ministry of Labor (URL: https:\/\/www.cardinal-tien.com\/NewMobileRegister\/ )<\/li>\n\n\n\n<li>Teachers, students and colleagues who have registered for the \"114 Annual Special Health examination for Laboratory Personnel\" at the Environmental Safety and Health Center are kindly requested to complete the appointment for the health checkup time before August 9. The appointment website is: https:\/\/reurl.cc\/WxerZe<\/li>\n\n\n\n<li>Please select a check-in period, and the maximum number of people in each period is 20. On the afternoon of August 20 (Tuseday), please check in at the Zhongzheng Hall according to the selected time period.
